The Norwood City Health Department assists the Ohio Department of Health by issuing vital documents such as birth certificates, death certificates and burial permits.
 

Facts

The Norwood City Health Department houses copies of birth and death certificates dating from 1908 to the present for those persons who were born or died within the Norwood city limits.  Our Registrar, Jean Tepe Tolbert, maintains those records and will assist you in finding documents either housed at our facility or provide information as to where your vital documents can be found.

The Norwood City Health Department issues vital records according to the laws and rules of the Ohio Revised and Administrative Codes.  All requests for vital records must be made in writing either at our facility or by mail.  All vital records must be certified by the local registrar.  We are not permitted to issue uncertified copies for genealogy research but are happy to assist you in reviewing our records on site.  Contact us at (513) 458-4600 for an appointment if you are planning a genealogy research visit.

Cost

Certified copies of both birth and death certificates at the Norwood City Health Department are $22 each.  This cost includes fees that we are required by law to collect on behalf of other agencies ($1.50 for Family Violence Prevention; $3.00 for Child Abuse Prevention; $5.00 for the Ohio Department of Health).  The Norwood City Health Department retains only $7.50 of the cost to maintain records, purchase vital statistics   paper and supplies and issue records.  Each local health district sets their fees for vital records storage and maintenance. 

Prior to burial or cremation, funeral directors are required by law to obtain burial permits.  The cost of burial permits is currently $3.00 and is forwarded to the Ohio Department of   Health less a 3% administration fee.

Payment for vital records at the Norwood City Health Department must be made by check, cash or money order.  We plan to offer payment by credit card in the near future.
