FACTS:
The Norwood City Health Department houses copies of birth and death
certificates dating from 1908 to the present for those persons who
were born or died within the Norwood city limits. Our Registrar, Jean
Tepe Tolbert, and Deputy Registrar, Donna Laake, maintain those
records and will assist you in finding documents either housed at our
facility or provide information as to where your vital documents can
be found.
The Norwood City Health Department issues vital records according to
the laws and rules of the Ohio Revised and Administrative Codes. All
requests for vital records must be made in writing (see “Application”
button to the right) either at our facility or by mail. All vital
records must be certified by the local registrar. We are not
permitted to issue uncertified copies for genealogy research but are
happy to assist you in reviewing our records on site. Contact us at
